Flourish Australia has multiple exciting opportunities for Transition Officers to join our brand new Queensland teams!
The positions available are:
- Full-time (76 hours/fortnight)
- Working Monday to Friday
- Based at either our Hervey Bay or Rockhampton sites
- Attractive salary starting at $85,000 per annum + Super + Tax-Free Salary Packaging.
About the Opportunities
The Transition Officer works in the community with people with lived experience of a mental health issue, supporting their access to Flourish Australia services and in pursuing their recovery goals. You will work closely with frontline staff, people Flourish Australia supports and key stakeholders to deliver the program’s objectives.
To be successful in this role you will have substantial administration experience, excellent communication skills, be proactive and able to work independently, with a thorough understanding of the mental health sector. You will:
- Provide administrative and telephone support
- Facilitate discussions regarding the individual needs and goals of each person accessing the service
- Be proficient in capturing personal information
- Support people to connect with other services as required
- Liaise with local managers
- Assist in the development of reports and projects
Please note - Successful candidates must maintain a Class C drivers license and proof of comprehensive insurance must be provided when required. You may be directed to use your private motor vehicle when an organisational pool motor vehicle is unavailable. You will receive the Travelling Transport and Fares Allowance when using your private vehicle for work related purposes.

Selection Criteria
Essential:
- Tertiary qualifications in human services or relevant field or a relevant Certificate IV in mental health related discipline
- Advanced administration skills and at least 2 years’ experience in a similar role as recognise by Flourish Australia.
- Demonstrated understanding of the challenges faced by people with a lived experience of a mental health issue and how providing a recovery-oriented service is of benefit to people.
- Demonstrated strong time management skills including prioritisation of competing tasks and an ability to work innovatively to solve complex issues.
- Demonstrated strong communication, advocacy and interpersonal skills including the ability to liaise effectively with a wide range of stakeholders, facilitate outcomes and prepare correspondence & reports in clear and concise language.
- Current unrestricted Australian drivers’ licence.
Desirable:
- A personal lived experience of a mental health issue or a carer of a person with a lived experience.

All appointments with Flourish Australia's Mental Health Services are subject to an NDIS Worker Screening clearance, a National Criminal History Check, a WWCC, previous employment reference checks, satisfactory Australian residency status and other background checks.
